As a parent/guardian, I give permission for my child to participate in the programs and activities of Cornerstone Youth Center. In addition, I give permission for Cornerstone staff/volunteer to transport my child if necessary by Cornerstone van:

This is a group for both guys and girls held weekly on Wednesday's, from 6:15-7:30pm at varying locations led by Jen Gibson and other Cornerstone Leaders. This is a special time set aside outside of youth hours because we want to be intentional with these young men and woman, to show them that they are valued and matter.

Realizing that transportation may be a problem for some, we are willing and able to assist in providing rides if needed, please let us know if this would ever be a need for your student.
