As a parent/guardian, I give permission for my child to participate in the programs and activities of Cornerstone Youth Center. In addition, I give permission for Cornerstone staff/volunteer to transport my child if necessary by Cornerstone van:

This will be a girl’s only group held weekly on Tuesdays, from 6:15 pm – 7:00 pm  led by Jenn and other female leaders. This is a special time set aside because we want to be intentional with these young women and show them that they are valued and matter. We will meet primarily in the Cornerstone cafe, and will occasionally go on fun trips, of which we will send permission forms for in advance.

Realizing that transportation may be a problem for some, we are willing and able to assist in providing transportation if needed, please let us know if this would ever be a need for your student.
